Before
you begin using your card |

Verify that the name on the card is
correct. |

In order to use your card, you must first activate
it. To activate your card, call the phone number on the sticker
located on the front of your card from your home phone, and follow
the instructions given to you. |

Please remove the sticker and sign your card immediately.
*In order to ensure the security of your ANA CARD U.S.A. credit card, when you make an in-flight purchase
on ANA domestic and international flights using your ANA CARD U.S.A., the cabin attendant will match your
signature on the back of the credit card to the purchase receipt.
In the interest of preventing credit fraud, if the cabin attendant is unable to verify your signature,
the purchases will not be approved. |

The credit limit of your account is shown on upper
portion of the card carrier. Please read the "Cardmember
Agreement" and place it in a secure place. |

To set up your own personalized Personal Identification Number (PIN) via an automated
system please call 800-733-5558. Or, contact customer service if you wish to received the PIN by regular mail.
We will mail you a pre-selected PIN for cash advance transactions upon request. You would receive your PIN
within 7 to 10 days after we receive your request.
