Apply online or through the mail by requesting to receive an application by email or telephone.

Please apply through the online application site below. Please type the campaign code [AU0210] into Campaign Code section on the application screen. Then please choose "Yes" for an additional card request and type an additional member's information.
* Applications without the campaign code will be deemed ineligible for the campaign.
* The applicant will be charged $10 as an application fee for an additional card.

Regarding Bonus Miles Accumulation

During the campaign period the applicant will receive 500 bonus miles when the application is approved and an additional card is issued. 500 Bonus Miles will be awarded to the account of primary card member by the end of May 2010.
* The bonus mileage for this campaign is limited to one bonus of 500 miles during the campaign period.
* If the credit card has been cancelled before the Bonus Mileage Accumulation, it will not be eligible for the campaign.

Only the residents of the U.S. are eligible for this campaign.
Upon applying for ANA CARD U.S.A. through internet, please enter the Campaign Code into Campaign Code Section. By application form, please write the Campaign Code on the "Remarks" section of the application. Applications without the campaign code will be deemed ineligible for the campaign.
Not valid with any other campaign or special offer.
Not eligible for this Additional Card bonus campaign after applied for ANA CARD U.S.A. or issuance of the card.
