Verifying your information

Once your application has been reviewed, you may be asked to call our service center to verify the information entered on the application.

Final Review

The final review will be made by the issuing bank, First National Bank of Omaha (FNBO®).

Complete

You will receive a notice in the mail letting you know if your application was approved or not. Please allow up to 3 – 6 weeks to receive this notice. If you are approved, please remember to activate your new card.

  • A. Income Statement (W-2 form, pay stubs)
    Please confirm that the document includes the issuance date, applicable period, gross income before taxes, applicant’s name, and the name of the issuing organization.
  • B. Employment Letter
    Please ensure that the document includes the issuance date, applicant’s name, gross annual income, address of the company/organization, signature of the preparer with the letter head of the issuer.
  • C. Proof of Financial Support Documents (for corporate-sponsored study abroad, grants, and scholarships)
    Please confirm that the document includes the issuance date, applicant’s name, amount of support, support period, contact information such as the name and address of the issuing organization, and the signature of the issuing authority.
